Mrs. Smith is a 55 year old female with a family history of Alzheimer’s, who recently discovered she alo has high blood pressure. Concerned about the effects her genetics and current lifestyle may have on her future cognitive health, she is interested in finding out what she can do to maintain a healthy brain.

1st appointment: Completes an Intake Interview with the Care Coordinator, answers questions about her medical, social, and family history, as well as information about her diet, daily activities and habits. She also brings in her medical records to go over with the Care Coordinator and fills out a SAM (Surgeries, Allergies, and Medications) form.

She then completes her Physical Assessment, and receives an introduction to CogniFit Software, learning how to navigate the program and generating her baseline assessment for easy-at-home use.

2nd appointment:
Completes Introductory Neuropsychological Assessment.

3rd appointment:
Has Team Meeting with relevant CFIT staff, is given Nutrition Options and Information, and receives her Personalized Program and results. Mrs. Smith is now ready to begin her home program!

  • Following her evaluations, Video Link-Skype and Conference call correspondence with CFIT staff is available, as needed, for one year.

  • Following her evaluation, Mrs. Smith may choose to continue visiting CFIT for their A-La-Carte services to aid her in her efforts to maintain a healthy lifestyle and fit brain. Click here to see a complete list of programs offered.

  • Alternately, Mrs. Smith may choose to upgrade to the Evaluation Services Package which includes a full neuropsychological evaluation with our Cognitive Psychologist, introduction to Wii, and Team Meeting with our Neurologist Kenneth S. Kosik, M.D.

    Mrs. Smith may realize that she would like more analyses done by our expert neurologist and staff and would prefer to join CFIT’s annual program, rather than trying to adhere to her program at home without CFIT’s support. (See Annual Program Package for details.)
