Mr. Smith lives in a town far away from Santa Barbara and has been a top-level executive his entire career. Due to some recent health issues, and driven by his desire to stay ahead of the game in his business ventures, he's begun to think of how to live a healthy life and stay as sharp as possible for as long as possible. He sees an article in the New York Times about a new cognitive research and remediation facility that has opened in Santa Barbara. Not being from the area, it is hard for Mr. Smith to commute back and forth. Thankfully, CFIT has a novel program that can be completed in just one day at their clinic.

Pre-Meeting:
Prior to his evaluation day, CFIT sends release forms and paper work for Mr. Smith to fill out, which he can mail or fax to CFIT. CFIT will also contact his physician's office to get a copy of his medical records and input them into our electronic health record system to ensure that everything is set in place for the big day.

Meeting:
CFIT sets up all your evaluations in a single stress-free day. This includes:

  • Medical Intake Interview: Mr. Smith completes the CFIT Intake Evaluation with the Care Coordinator, and has a brief Medical Examination. Mr. Smith's medical records will already be filed in our electronic health record system.

  • Neuropsychological Testing and Assessment: Completes Neuropsychological Evaluation given by our Cognitive Psychologist to determine level of cognition.

  • Lunch: Delicious local cuisine is catered for both lunch and refreshments.

  • Physical Assessment: Meets with Occupational Therapist to complete Physical Assessment, receives introduction to Cognifit to learn how to navigate the program and generate baseline assessment for home-use, and receives an introduction to the Wii console, including a description of benefits and information on where to purchase one.

  • Refreshments: A short break is given to relax and enjoy some refreshments while the Team discusses the outcomes from the day’s assessments and begins to develop your Personalized Program.

  • Team Meeting: Mr. Smith will then have his Team Meeting with all relevant medical practitioners and receive an overview of his Personalized Program and results. Mr. Smith’s family or other practitioners are encouraged to attend this meeting and Video-Link Skype and Conference calling is available for out-of-area family members and doctors.

  • Following the evaluation: Mr. Smith will receive a detailed copy of his Personalized Program and letter in the mail. He will also receive one year of Cognifit service, as well as e-mail and telephone guidance from the Care Coordinator, when needed. CFIT staff conducts detailed correspondence with Mr. Smith’s hometown physicians and other service providers upon request. Video-Link Skype and Conference call correspondence with CFIT, as needed, for one year.
