Mr. Smith has begun to worry about things he can't seem to remember: names, where his car keys are, when he last saw a good friend, appointments he had scheduled. His wife has become increasingly concerned about his memory and knows he has family members with Alzheimer's and other dementias. She has recently learned that new research may slow down the onset of cognitive diseases and that a new clinic in town can help.

1st appointment: Mr. Smith meets with our Care Coordinator for his Medical Intake Interview where he answers questions about his medical, social, and family history, as well as information about his diet, daily activities and habits. Before he comes in, CFIT has contacted Mr. Smith's physician's offices to get a complete copy of his medical records and has input them into the electronic health record system.

2nd appointment: Neuropsychological testing is given to determine his level of cognition by our Cognitive Psychologist.

3rd appointment: He meets with our Occupational Therapist to complete his Physical Assessment, for an introduction to Cognifit to learn how to navigate the program and generate his baseline assessment for easy at-home use, and an introduction to the Wii console with a description of benefits and information on where to purchase one.

4th appointment: Mr. Smith has his Team Meeting with all relevant medical practitioners and receives his Personalized Program and results. He is now ready to begin his home program!

• Over the following year, Mr. Smith receives one year of Cognifit service, as well as e-mail and telephone guidance from the Care Coordinator, when needed. CFIT staff conducts detailed correspondence with his physicians and other service providers, upon request. Video Link-Skype and Conference call correspondence with CFIT staff, as needed, is also provided for one year.
  
  
• CFIT provides ongoing A-La-Carte services which are available to Mr. Smith at any time. If he feels like he needs support in a specific area (nutrition, social services, information on new technology, etc) our facilities are always open for his use. • Alternately, Mr. Smith may realize he would like more analyses done by our expert neurologist and staff and would prefer to join CFIT's annual program, rather than trying to adhere to his program at home without CFIT's support. (See Annual Program Package for details.)
