Mrs. Smith is in excellent health but has read the statistics of Alzheimer's disease and has become a little more forgetful recently. As a 62 year old she realizes that she is at risk for the disease and also takes care of her ailing mother, who depends on her to be sharp. She wants to learn about Alzheimer's disease and other dementias and what she can do to delay its onset or slow its course.
1st appointment: Mrs. Smith completes the CFIT Intake Evaluation with the Care Coordinator answering questions about her medical, family, social history and providing information about her diet, daily activities and habits. CFIT contacts Mrs. Smith's physician's office to get a copy of her medical records and inputs them into our electronic health record system.
2nd appointment: In preparation for entry into the CFIT program Mrs. Smith completes neuropsychological evaluation given by our Cognitive Psychologist to determine her level of cognition.
3rd appointment: Mrs. Smith comes to meet with our Occupational Therapist for an introduction to CogniFit to learn how to navigate the program and generate her baseline assessment for home and clinic use, an introduction to Wii and the NuStep, and completes the Physical Assessment.
4th appointment: Mrs. Smith has her Team Meeting with all relevant medical practitioners and receives her Personalized Program and stops by the front desk to make upcoming weekly appointments!
- Over the ensuing year Mrs. Smith visits CFIT for a program of physical exercise and cognitive remediation about one time per week for about one hour each time. After one year the neuropsychological testing is repeated, the change in performance is compared with CFIT's database. A complete report is generated and a family conference is set up with CFIT staff.
- Mrs. Smith may also utilize the other A-La-Carte programs offered by CFIT if there is a specific area she would like to address, whether it concerns nutrition, genetic testing, social services, end- of -life counseling, information on current memory devices, areas of new research, etc. Click here to see a complete list of programs offered.
